I, the owner, accept that while Lauren Fonte is adequately qualified for this service and will take reasonable precautions, the nature of dog interaction & working in unpredictable, real world environments carries inherent risks and means injuries can occur. If my dog receives any unlikely injury during the session that requires treatment by a vet, I understand this will be covered by The Holistic Hound Dog Training's Business Insurance (MGA Insurance Group). I understand that any vet treatment not absolutely necessary or immediately linked to the injury occurring during the session will not be covered.
I understand and accept that if any unlikely incident was to occur whereby my dog has caused injury to another person or animal, that I will need to comply with local dog laws and processes and pay costs if necessary. I hereby release and agree to hold harmless Lauren Fonte of The Holistic Hound Dog Training of any and all liability, costs, injury, damage or loss of any nature, or for any liability, costs, injury, damage or loss which may be caused in any way by my dog(s), whereby all reasonable precautions were taken to avoid incident & injury.
While my dog(s) is/are in the care and custody of Lauren Fonte of The Holistic Hound Dog Training, if I am unreachable in the event of an emergency, I hereby authorize Lauren Fonte of The Holistic Hound Dog Training, to seek immediate veterinary care from the nearest or best available Vet Clinic for my dog if required.
I understand that Lauren Fonte of The Holistic Hound Dog Training warrants to keep safe and confidential all keys, remote control entry devices, access codes and personal information of the Owner and to return to the Owner at the end of the contract period or immediately upon demand.
The Owner is the person filling out/signing this form, 'my dog' is the dog noted in this form which is to be taken out for the session.
